Aesthetic surgery · body contouring after weight loss · the extent of an excision
Dr. Tongchun Mao is an aesthetic surgeon within the Plastic Surgery & Aesthetics Medical Center at Chongqing West District Hospital. Within the team his role is refinement and honest referral: he is the person who says so when an operation will do better than an injectable or a device.
How far the skin has stretched is the whole of this decision, and Dr. Mao's part of it is the examination that measures it. He looks at the abdomen, then follows the laxity round the flanks to the back, the hips and the buttocks, and asks how the skin behaves when it is lifted rather than how it looks lying down. Where the complaint is a firm abdomen that protrudes because of fat behind the muscle, or skin that has stretched while weight is still moving, his answer is that the operation is the wrong one for now — and he says so before any plan is discussed.
Clinical focus
The scar is a decision, not a side effect
A tummy tuck leaves a scar low across the abdomen. A body lift leaves a scar that goes all the way round, because that is where the skin came from. The two are the same operation applied to a different extent, and the extent decides the scar. Nobody should accept a circumferential closure without accepting a belt-line scar, because they are one decision rather than two.
